Real tilt shift lenses are expensive. Lensbaby, for example, do tilt lenses intended for "miniaturisation" effects: http://lensbaby.com/lenses-composer-pro-edge80

The lenses are REALLY expensive, so most people do photoshop like /u/hydrogenjoule said. The lens has the ability to turn at an angle, which is what creates the banded (linear?) blurring.

Either. There is a special lens, but mostly these days the effect is done in Photoshop. Tilt shift lenses have other purposes, too, not just miniaturization.
